Submit a Claim - Instructions

If you have used the Uber Application as a Driver in California or Massachusetts between August 16, 2009, and February 28, 2019, and you are not bound by Uber’s arbitration clause (either because you validly opted out of arbitration or because Uber has no record of your acceptance of an arbitration agreement), you could get a payment from this Settlement. The only way to get a payment is if you submit a Claim Form. The online Claim filing deadline is August 17, 2019 at 11:59 p.m. PST, but you should submit your claim as soon as possible. Claim Forms submitted by mail must be postmarked on or before August 17, 2019.

No matter which method you choose to submit your Claim Form, please read the Claim Form carefully and provide all the information required. Only one Claim Form may be submitted per Claimant.

How to Submit by Mail

If you received a mailed Notice, a Claim Form was included. You can also download a copy here, or contact the Settlement Administrator at 1-855-571-5869 or to get a copy sent to you. Please read the form carefully and provide all required information. Please send your completed Claim Form to the Settlement Administrator via U.S. Mail to the following address:
